DESCENTRALIZAÇÃO DO SISTEMA ÚNICO DE SAÚDE: AS DIFICULDADES DE O MUNICÍPIO PROGRAMAR

Este trabalho busca refletir sobre as dificuldades que levam o município a se programar e fazer cumprir o Direito Constitucional à Saúde de maneira mais efetiva. Para tanto, utiliza-se de revisão bibliográfica sobre o tema e da análise qualitativa de entrevista semi-diretiva realizada com um Secretário Municipal de Saúde, denominado E1. Apresenta-se um breve resgate histórico do processo de construção do Sistema Único de Saúde, da descentralização e de seu aporte jurídico. Na sequência, destacam-se algumas dificuldades que os municípios encontram na gestão descentralizada e na busca por um novo modelo de atenção, na visão de um Secretário Municipal de Saúde. Conclui-se que o atual modelo de atenção e principalmente o modelo de gestão do governo são sem dúvida pontos chaves que dificultam a programação do Município e a garantia da efetividade do direito à saúde a todos os cidadãos. O governo tem que servir ao cidadão e não o cidadão servir ao governo.

ABSTRACT

This paper seeks to reflect on the difficulties that lead the council to set and enforce the constitutional right to health more effectively. Therefore, we use literature review on the subject and qualitative analysis of semi-directive performed with a Municipal Health Secretary, called E1 interview. Presents a brief historical review of the Unified Health System (SUS), decentralization and its legal input process. Following, we highlight some difficulties that municipalities are in decentralized management and the search for a new model of care, the vision of a Municipal Health Secretary. It is concluded that the current model of care and especially the management model government are undoubtedly key points that complicate programming Municipality and ensuring the effectiveness of the right to health for all citizens. The government has to serve the citizen, not the citizen serve the government.
