MANEJO CLÍNICO DE SEPSE GESTACIONAL: UMA REVISÃO SISTEMÁTICA DA LITERATURA

Resumo


Objetivo: Explorar as abordagens utilizadas para diagnóstico e tratamento de sepse materna em estudos recentes, e relatar os resultados similares.

Metodologia: Estudo de revisão sistemática utilizando as bases de dados LILACS, BVS, Pubmed/Medline e Cochrane. A pesquisa foi conduzida considerando os termos “sepsis”, “septic shock” e “pregnancy”. A qualidade dos artigos foi avaliada usando Study Quality Assessment Tool from the Department of Health and Human Services (NHLBI).

Resultados: Um total de 236 estudos foram identificados 8 estudos foram elegíveis e incluídos. A metodologia observacional do tipo coorte retrospectiva foi predominante, e mais da metade dos estudos (n=6) consideraram a presença de 1 ou mais sintomas clínicos e achados laboratoriais para diagnóstico de sepse materna, porém não foi descrito a frequência da sintomatologia e resultados laboratoriais predominantes. Iniciar o tratamento com antibioticoterapia intravenosa dentro de 1 hora do reconhecimento de sepse foi priorizado em 6 estudos, seguido de hidratação com volume/fluídos de ressuscitação e drogas vasopressoras (n=4), cultura para identificação do agente etiológico (n=3) e admissão em unidade de terapia intensiva (n=3). Morte materna (n=7) e fetal (n=3), seguido de prematuridade pré-natal (n=2) foram as principais complicações obstétricas.

Contribuição: Critérios padronizados para diagnóstico precoce e manejo de sepse materna ainda não foram evidenciados entre os estudos, e as justificativas giram em torno das modificações fisiológicas, imunológicas e mecânicas que acontecem durante a gestação e puerpério. Essas alterações dificultam o reconhecimento da sintomatologia relacionada a um quadro de sepse e atrasam o diagnóstico e medidas de tratamento.

Results: A total of 236 studies were identified 8 studies were eligible and included. The observational methodology of the retrospective cohort type was predominant, and more than half of the studies (n=6) considered the presence of 1 or more clinical symptoms and laboratory findings for the diagnosis of maternal sepsis, but the frequency of symptoms and laboratory results were not described. predominant. Initiating treatment with intravenous antibiotic therapy within 1 hour of recognition of sepsis was prioritized in 6 studies, followed by hydration with volume/resuscitation fluids and vasopressor drugs (n=4), culture to identify the etiologic agent (n=3) and admission to the intensive care unit (n=3). Maternal (n=7) and fetal death (n=3), followed by prenatal prematurity (n=2) were the main obstetric complications.

Contribution: Standardized criteria for early diagnosis and management of maternal sepsis have not yet been evidenced among the studies, and the justifications revolve around the physiological, immunological and mechanical changes that occur during pregnancy and puerperium. These changes make it difficult to recognize the symptoms related to sepsis and delay diagnosis and treatment measures.
